2.1 Sports Physicals. OSF shall cause one or more qualified providers employed or contracted by OSF or an affiliate of OSF to be available on-site at SCHOOL to provide pre- participation sports physicals (“Sports Physicals”) one (1) time per year, or as otherwise agreed upon by the Parties, at such dates and times as mutually agreed upon by the Parties. As Sports Physicals shall be administered on school premises, SCHOOL shall provide the necessary rooms, dividers, and other items requested by OSF to perform the Sports Physicals. Sports Physicals shall include the following components provided by licensed providers, as specified: (1) height and weight; (2) vision screening; (3) heart rate and blood pressure check; (4) sports exam by a licensed provider, or (5) any other component mutually agreed upon by the Parties in writing prior to the upcoming School Year. Sports Physicals shall be administered in accordance with OSF policies, practices and procedures; OSF and SCHOOL acknowledge and agree that the provision of Sports Physicals by OSF to any SCHOOL student athlete shall be conditioned on the receipt by OSF of any consent(s) and/or authorization(s) deemed necessary by OSF. 2.2 Training Services and Supplies. OSF shall provide on-premises athletic training services (“Training Services”) of an athletic trainer employed by or contracted by OSF to student athletes up to EIGHT HUNDRED AND FIFTY (850) hours per school year, or as otherwise agreed upon by the Parties, at such dates and times as mutually agreed upon by the Parties. Training Services shall include baseline and post-injury concussion testing as appropriate. Training Services shall be provided in accordance with applicable law. Minimally, OSF shall provide Training Services at: (i) all varsity football games (both “home” and “away” games, as well as playoff games), (ii) SCHOOL’s “home” basketball, baseball and softball games, (iii) SCHOOL’s playoff basketball games, and (iv) such other such other sporting practices and/or events as are requested by SCHOOL and agreed upon by OSF. Notwithstanding the foregoing, the parties acknowledge and agree that on-field coverage of football games shall take precedence over on-field coverage of other sporting events. OSF and SCHOOL acknowledge and agree that the provision of Training Services by OSF to any SCHOOL student athlete shall be conditioned on the receipt by OSF of any consent(s) and/or authorization(s) deemed necessary by OSF. SCHOOL shall provide OSF with access to SCHOOL facilities and with the necessary supplies and information needed to perform competent Training Services. SCHOOL shall provide all expendable medical supplies, tape, etc., and the storage of said supplies. If requested by SCHOOL, the athletic trainer will assist in ordering the supplies, subject to any budget imposed by SCHOOL.

2.3 Billing for Sports Physicals. There shall be no payments between OSF and SCHOOL associated with the Sports Physicals services under this Agreement.
