Students will be charged Twenty Five and no/100 Dollars ($25.00) per Sports Physical. This must be paid to OSF at the time the Sports Physical is performed.

2.4 Payment for Training Services. In exchange for the provision of Training Services by OSF hereunder, SCHOOL shall pay to OSF the annual sum of EIGHT Thousand and no/100 Dollars ($8,000) per school year. 2.5 Invoicing for Training Services. OSF shall submit an invoice to SCHOOL on the 1 st of August each year, for the full amount due for Training Services for the upcoming school year. SCHOOL shall make payment to OSF within thirty (30) days of receipt of invoice. 2.6 Limitation of Services Covered by this Agreement. SCHOOL specifically acknowledges that this Agreement is limited in scope to the Services specified in Sections 2.1 and 2.2 of this Agreement, and further acknowledges that, except for the Sports Physicals contemplated under Section 2.1 herein, the provision of professional medical services by an OSF physician or other provider to a SCHOOL student athlete, or any other service that may be provided by OSF or any affiliate of OSF to a SCHOOL student athlete, is outside of the scope of this Agreement. In the event a SCHOOL student athlete obtains the services of an OSF physician or other provider at an OSF facility or otherwise, OSF shall have the full right and authority to bill for such services. 3.1 Meeting Attendance. OSF shall be entitled to have one (1) or more representatives attend all: (i ) SCHOOL meetings related to SCHOOL’s athletic programs, team sports, health and wellness, and any other matter agreed upon by the Parties; and (ii) SCHOOL m eetings with Students’ parents related to athletic programs, team sports, health and wellness, and any other matter agreed upon by the Parties. 3.2 Marketing, Advertising, and Promotional Activities. As additional consideration for the provision of Services by OSF hereunder, during the term of the Agreement, SCHOOL will provide OSF with the following marketing, advertising, and promotional activities, including all production costs (unless otherwise indicated): SPONSORSHIP, MARKETING, AND ADVERTISING SERVICES.

a. Signage at all SCHOOL sports venues (banners and/or signs to be provided by OSF);

b. Advertisement in programs for any SCHOOL sporting events or other documentation associated with SCHOOL athletic programs whereby SCHOOL offers advertising (SCHOOL to coordinate with OSF on advertisement which must be approved by OSF);
