STUDENT ACCIDENT INSURANCE Student accident insurance is available for purchase by every student enrolled at PTHS. Athletes are required to carry a policy or have an insurance waiver on file. According to a schedule of benefits listed in the policy, some expenses incurred in an accident suffered while in school, while in an authorized school sponsored trip, and while traveling to or from school are covered. If you have school insurance and sustain an injury, report it at once to the office and obtain a claim form. It is the student’s responsibility to take the claim to the doctor and see that it is processed. LOST AND FOUND A lost and found area will be maintained in the main office. Students are encouraged not to bring valuables or excessive amounts of money to school. All items unclaimed at the end of the school year will be donated to charity. NON-CUSTODIAL REQUEST FOR RECORDS If a non-custodial parent would like to receive copies of report cards, progress reports and discipline referrals, they need to submit a written request. The request should be directed to the Principal. ASBESTOS NOTICE TO ALL STUDENTS, EMPLOYEES, AND PARENTS In accordance with federal regulations (45FR61987) Pontiac High School is required to notify all students, employees, and parents, that friable asbestos is present in some areas of the building. A program has been implemented to ensure that the material is maintained in good condition and that appropriate precautions are followed. PESTICIDE AND LAWN CARE APPLICATION NOTICE The district maintains a registry of parents and guardians of students who have registered to receive written or telephone notification prior to the application of pesticides and lawn care products to school grounds. To be added to the list, please contact the school. TEACHER QUALIFICATIONS Parents/guardians may request information about the qualifications of their student’s teachers and paraprofessionals, including: ● Whether the teacher has met State licensing criteria for the grade levels and subject areas in which the teacher provides instruction; ● Whether the teacher is teaching under an emergency or other provisional status through which State qualification and licensing criteria have been waived; ● Whether the teacher is teaching in a field of discipline of the teacher’s certification; and ● Whether any instructional aides or paraprofessionals provide services to your student and, if so, their qualifications. If you would like to receive any of this information, please contact the school office. Notification will be given before application of the pesticide and lawn care products. Prior notice is not required if there is imminent threat to health or property
