RANDOM DRUG TESTING All students who want to participate or try-out must sign the consent form for random drug testing before they will be allowed to participate in an IHSA activity. Students deciding to participate in an extracurricular activity after the sign-up deadline will be required to be tested after the signed consent form is returned. The cost of the test will be the responsibility of the student/athlete. MEDICAL/INSURANCE FORMS Completed physical, medical release and proof of insurance must be on file before a student will be allowed to try out or participate. SUPERVISED STUDY PROGRAM Students who receive two “D’s” or one failing grade (F) through their last contest will be required to attend three supervised study sessions each week until their grade/s have been raised. Information regarding location and time can be found on the athletics website. The following consequences will be in effect for students who fail to attend the required study sessions in a week: 1st Offense: Student will meet with the Athletic Director and/or tutoring supervisor. 2nd Offense: Student will be ineligible for the next contest. 3rd Offense: Student will be ineligible for one week of contests. 4th Offense: Student will be dismissed from the team. *The count of offenses will reset at the start of each athletic season (fall, winter, spring)* TRAVEL All athletes shall travel to athletic events with the team on which the athlete competes by use of school approved means of transportation. We believe it is valuable for members of a team to celebrate a victory or comfort each other in defeat, so all athletes shall return home from athletic events with the team on which the athlete competes by use of school approved means of transportation. If a program’s head coach allows waivers of this rule, a sign out sheet will be available at the team’s event and: 1. Parents or guardians (as identified by the school’s information system) may sign out their student-athlete without advance notice, OR 2. A student-athlete may be signed out by and ride home with a teammate’s parent/guardian OR an adult relative if the student-athlete’s parent/guardian provides written notice (note or email) to the coach prior to the team’s scheduled departure time.

Any student-athlete found to be in violation of this policy shall be subject to discipline in accordance with the school district’s athletic discipline policies.
