Maternity Leave Request

Dear Dr. Ryan McGuckin, I am writing to formally request maternity leave starting on my due date, February 23, 2025, with an anticipated return date of May 18, 2025. These dates equate to 54 school days/teacher days missed. I would like to use 16 sick/personal days, leaving 38 dock days. I have made arrangements in my office to ensure a smooth transition during my absence, including a complete substitute binder with all the necessary information required to run the nurse’s office while I am away. I will have my FMLA paperwork and doctor’s note regarding my pregnancy after our next appointment on November 3, 2025. Please let me know if you need any more documentation from my healthcare provider or if there are any additional forms I should complete.
