D. Social Worker and Other CMT members will: 1. Meet as a team to gather information, to assess the situation, and to determine the degree of professional mental health intervention necessary in the building; 2. Assist in the planning of intervention strategies; 3. Designate a team member to act as school/home liaison;

4. Monitor students determined to be at-risk and notify parents; and 5. Return the building to normal operation as soon as possible.

E. The Community Mental Health Liaison will: 1. Meet with the CMT to assess the situation and determine needs; 2. Notify community agency and be prepared to assign agency personnel to the high school; and 3. Assist in the intervention and prevention process.

III. Pontiac Township High School District #90 shall provide staff development with the warning signs for at-risk students and procedures to be followed after a suicide or death.

IV. A member of the CMT team will be responsible for evaluating and revising this plan annually.
